WISeR’s rollout delayed care and produced troubling denials; the full scale of AI-caused harm still needs evidence.
WISeR is best understood as a rushed prior-authorization pilot whose technical failures, weak communication, and vendor-management problems disrupted care. The records support real delays and inappropriate denials, but they do not establish that every denial came from an AI model or provide a complete measure of patient injury. Records show delays, technical failures, inaccurate denials, and weak communication. The contract made rejection profitable, paying vendors up to a quarter of a service’s typical Medicare payment while slow answers faced no comparable penalty.
