The Story
The brainstem can keep consciousness switched on, but the felt contents of experience still appear to depend on cortical interaction.
The strongest ordinary reading separates wakefulness from phenomenal content. Brainstem and thalamic circuits help regulate the conscious state and shape perception, but evidence linking experience itself to cortex remains substantial. The 2025 thalamic study points to a thalamofrontal loop, not an independently conscious deep-brain generator; the central question is whether interaction, rather than location, is what matters.
- Clinical stakesDisorders of consciousness should be studied as failures of interacting brain systems, not as damage to one isolated center.
- Evidence boundaryDeep-brain involvement is better established than deep-brain independence.
- A 2013 critique argues that wakefulness should not be confused with conscious contents and reports converging evidence for cortical involvement in awareness.
- A 2007 review concluded that the midbrain reticular system is crucial for waking, while phenomenal consciousness involving experience and perception appears to require the thalamocortical system.
- The 2025 human study linked conscious detection to information flow from medial and intralaminar thalamus to prefrontal cortex through a thalamofrontal loop.

Deeper Insight
Devor and Baron, the paper's authors, put feeling below thought. A silent brain may be more awake than its language lets on.
The paper starts with pain and points downward, arguing that primitive brainstem circuits may generate the basic feeling while the cortex adds detail. Other evidence ties awareness to brain-wide loops, not one hidden command center. Still, the proposal exposes a dangerous gap: we may mistake lost speech for lost experience.
- What it could changeMedicine could search for hidden awareness below lost speech when assessing pain and disorders of consciousness.
- How it touches todayThalamic research already shows deep brain circuits helping gate what reaches awareness.
- Our futureA damaged cortex could stop telling the story while a buried feeling still continues.
- The originating hypothesis proposes that older brainstem circuitry generates the basic capacity for subjective experience, using pain and other inputs as a central clue.
- Antonio Damasio's account places consciousness in the brainstem and links feelings to bodily regulation, including pain, hunger, and wellbeing.
- A thalamic dynamic-core theory proposes that primary conscious awareness arises from synchronized activity in dorsal thalamic nuclei, while acknowledging links to cortical computation.
